Are you passionate about making a positive and lasting difference for the younger adult persons of Hull?
The Adult Social Care vision in Hull is ‘a life not a service’. Adult Social Care is person centred and focuses on individuals’ strengths to support them to take control of improving their own health, resilience and wellbeing. We want to maximise people’s independence so individuals can achieve their goals and aspirations and live life to the fullest. We work creatively with individuals, local communities and our partners, empowering them to deliver the best possible outcomes for people and to create a positive Adult Social Care culture. Our work is solution focussed, ensuring that those who most need support receive it and that we can continue supporting people in the future
Are you passionate about making a positive and lasting difference for young people transitioning into adulthood in Hull? There is an exciting opportunity available - We are looking to appoint a Grade 9 Specialist Transition social workers to be part of a newly formed Preparation for Adulthood Team (Pfa) based within the High Needs Team in Adult Social Care.
If you are inspired by a challenge and want to make a lasting difference, to support younger adults with diagnosis of Learning Disability, Autism or Mental Health we look forward to hearing from you.
You will work as part of an integrated multi-disciplinary team to provide high quality social work practice. You will ensure individuals have a solution focussed, strength-based plan towards achieving their goals and maintaining their independence, throughout their journey transitioning into adulthood.
You will work creatively and collaboratively with younger persons (in some cases from the age of 16) with our partners in Children’s services. This is to provide a consistent approach within a social work interface in preparation for their transitioning into adult’s services.
The team will follow a Multi-Disciplinary approach working with our partners in the Children’s Disability Teams, Education and special educational needs (SEND), Child and Adolescent Mental Health Services (CAMHS), alongside health professionals, probation, criminal justice, housing and commissioning bespoke provisions with our commissioning managers; to deliver the best possible outcomes for younger persons to create a positive introduction to our Adult Social Care culture.
We are looking for individuals who are qualified and experienced social workers who can demonstrate:

a strong value base that informs your work with young persons and adults.
a continuing commitment to quality outcomes - and to be able to talk about what good Social Work practice 'looks like'
a capacity for honest reflection and openness to learning.
Ability to establish professional, effective working relationships with a range of partners/colleagues, adults, families and their carers
The ability to work in a multidisciplinary arena to promote autonomy and implement personalised safeguarding
Understand the complexities involved with vulnerable younger persons.
Experience of supervising staff and supporting AYSE social workers and providing mentoring role for apprentices and trainee social workers
Experience with Court of protection work, community Deprivation of Liberty and 21a Challenges.
Have experience of the criminal justice system

Work as part of a team and can work autonomously and have the ability to make complex decisions for individuals who are vulnerable.

You will hold a professional social work qualification registered to Social Work England and will have experience of assessment and planning, initiating, monitoring, developing and providing outcomes-driven services in adult social care.
We provide a range of benefits to motivate, reward and recognise the valuable work of our employees, which include:

Work/Life Balance - The Council recognises that all employees need to balance home life with meeting work commitments through the provision of appropriate policies and support, for example, family and parental leave, flexible hours and career break schemes.
Learning and Development - The Council has a commitment to lifelong learning. We provide excellent development opportunities for all employees.
Pension - We provide a defined benefit pension scheme whereby the Council contributes towards a pension payable on retirement
Employee discounts - There are a selection of voluntary benefits available giving a wide variety of staff discounts on goods and services from a range of suppliers via our Hull plus website
Relocation Allowance available to minimise any financial detriment which may be incurred by newly appointed staff moving from outside the area
Laptops and telephone for all social workers, to enable them to work smarter and more flexibly

You must be able to provide and support the delivery of high quality social work activities, however, we have opportunities for different levels of experience as follows:
This post is for you if you have completed your ASYE and can demonstrate increased decision-making autonomy in situations of increasing complexity, risk, uncertainty and challenge.
The High Needs Team provides consistent and intensive case management during the initial stages of a service being set up or where the professional view is that support can be reduced over a period of time e.g. over the first years of care. It provides a level of assurance for the management team (DMT) in terms of having oversight across packages of support which may involve a community deprivation of liberty and where there is a requirement to regularly review the restrictions which are in place to keep the person safe.
For an informal discussion about the role, please contact Deborah Haytree Operational Manager on 01482 614209 or Deborah.haytree@hullcc.gov.uk
This role is not available for sponsorship therefore is only open to applicants who have the right to work in the UK
