An exciting opportunity has arisen to join our established and highly respected Critical Care Outreach Team. The team delivers a 24/7 service across Stoke Mandeville and Wycombe General Hospitals at Buckinghamshire Healthcare Trust.

We are looking for a highly motivated, innovative and critical care practitioner to deliver patient-focused care to acutely ill, deteriorating and critically ill patients across the trust. The successful candidate will need to display excellent communication, interpersonal and leadership skills, be able to respond to rapidly changing, high pressure situations and priorities. You will be able to work autonomously and as part of a team and have good critical thinking and decision making skills. You will be instrumental in the early recognition of deterioration and deliver an effective response, liaising with the patient’s own team. You will be able to facilitate smooth transitions of care for patients stepping down from Intensive and High Dependency Care to the general wards and be involved in the provision of a range of education for the multidisciplinary team.
• Ensure that appropriate, consistent and realistic information is provided to patients and families, seeking guidance from respective home teams when required.
• Ensure confidentiality is maintained without compromising patient care.
• Facilitate effective communication between patients, their families and the multidisciplinary teams (MDT).
• In collaboration with the home team, address sensitive issues surrounding futility of continuing treatment and the appropriateness of resuscitation decisions.
• Develop all staff to communicate and escalate concerns regarding ward patients effectively using a recognised communication tool e.g. SBAR
• Liaise and communicate with the Clinical Site Practitioners and the Hospital at Night Team in the management of acutely ill patients on the ward.
