Crynodeb
Act as a role model to colleagues, always seeking to maintain the highest standards of professionalism. Use initiative and take responsibility for themselves and the quality of their work and the service provided to patients. Lead, coordinate and develop the provision of an effective and efficient service for CYP. Provide day to day leadership, support and supervision of the staff within the MHSTiS as well as supporting the service lead to provide a seamless service. Support the effective triaging of patients referred to the teams, supporting the co-ordinated care delivery across the patient pathway. Provide expert clinical holistic assessment and evidence based pro-active and reactive interventions to support patients and their carers. Lead the teams in the implementation of integrated MDT care planning ensuring patients receive high quality, patient-focussed care at all times.
