1. Promote and value the rights, responsibilities and diversity of people. 2. Promote and maintain effective communication with people in order to build and sustain positive working relationships with individuals, families, friends and carers, team members and other agencies. This may include accessing translation and interpreting services. 3. Contribute to the planning, implementation and review of care, support and treatment, including implementing specific parts of an individual support plan on behalf of different care professionals. 4. Promote continuity of care by maintaining regular contact with service users during episodes of inpatient treatment, liaising closely with the ward/hospital and helping to facilitate early discharge. 5. Maintain accurate, confidential records of patient activity, writing reports and letters. 6. Enable people to understand and manage their mental health needs in line with their support plan and relapse prevention strategies. This may include enabling people to manage their own domestic and personal resources, enabling people to access learning / training, or work opportunities and helping people with their housing needs. 7. Support individuals in developing and maintaining their identity and personal relationships, including sometimes difficult or potentially difficult relationships. 8. Enabling people to explore a range of behavioural responses and to develop coping strategies in liaison with appropriate specialist teams. 9. Enable people with Mental Health needs to undertake their own health care, in accordance with their support plan. 10. Contribute to the protection of individuals from abuse and harm in line with local policies and procedures. 11. Develop own knowledge and practice, making use of supervision and available learning opportunities. 12. Promote, monitor and maintain health, safety and security in the workplace. Policies and Procedures Trust employees are expected to follow Trust policies, procedures and guidance as well as professional standards and guidelines. Copies of Trust policies can be accessed via the staff intranet (Ourspace) or from your manager. In particular, attention is drawn to the Trusts arrangements in relation to safeguarding children and vulnerable adults as well as infection prevention and control. All employees are expected to be familiar with the Trusts approach to risk management, take a risk management approach to their own work and take responsibility for the management of the risks they own.
