About the Role: As a Social Worker, you will provide essential support to individuals and families facing a wide range of challenges. You'll assess needs, develop care plans, and work alongside multidisciplinary teams to safeguard vulnerable people and improve their quality of life.

Key responsibilities include:
* Comprehensive Assessment: Conduct detailed assessments and risk evaluations for service users.

* Advocacy: Support clients to access healthcare, housing, benefits, and community resources.

* Documentation: Maintain accurate and timely case notes in compliance with professional standards.
