1. Clinical/Operational Management 1.1. Utilises advanced specialist knowledge covering a range of procedures and underpinned by relevant broad-based knowledge, experience and competence to provide care for patients with complex needs 1.2. Provide operational leadership and line management of their defined service area taking into account issues impacting internally and externally; proactively seeking creative solutions and sharing learning openly. 1.3. Lead multi-discipline work streams; the post holder will propose, develop and implement clinical and/or strategic policies and/or service developments; contributing to the planning and management of impacts on other disciplines, services or agencies including integration agendas across the local health and social care economy. Assess and assimilates local health needs, leading service development to ensure long term objectives are met 1.4. Manage the budget for the service and support the Clinical Director to ensure that the PCN budget is spent in accordance with the PCN DES Specification. Takes responsibility for their own and others health and safety in the working environment including compliance with health and safety standards throughout the area of responsibility 1.5. Ability to make highly complex judgements, including in new or unfamiliar situations by analysing and interpreting a wide variety of complex information and data sources to compare options and to take action as appropriate (this may be clinical or non-clinical judgements). 1.6. Deliver information to the team and other services, teams or agencies using a variety of means ensuring that communication networks are robust and fit for purpose. Promotes and represents service at a strategic and clinical level both internal and external to the organisation 1.7. Provides and receives highly complex, highly sensitive or highly contentious information where there may be barriers to acceptance which need to be overcome using persuasive, motivational or negotiating skills. 1.8. Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of the role including the management of compliments and complaints. 1.9. Responsible for ensuring all developments align to the clinical pathways already established in the system, and the correct links are made to ensure fluid and joined up working. 1.10. Engages and actively involves the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of self care, involvement and empowerment 1.11. Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of the role including the management of compliments and complaints 1.12. Works to standards of advanced proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a highly specialist standard 1.13. Acts as a highly specialist expert source of information and support for other professionals and agencies related to their field of speciality. 1.14. Has the position as Named Lead for safeguarding in the PCN and will ensure that training is kept up to date for all Nurse and GPs. 1.15. This role will establish a Childrens and Young Persons Social Prescribing service in the PCN. They will be the Clinical Lead for this service and be responsible for the operational delivery of this service. 2. Management 2.1. Provide operational leadership and line management of their defined area taking into account issues impacting internally and externally; proactively seeking creative solutions and sharing learning openly. 2.2. Lead multi-discipline and multi-agency work streams; the post holder will propose, develop and implement clinical and/or strategic policies and/or service developments; contributing to the planning and management of impacts on other disciplines in the PCN 2.3. Ensures the effective and efficient use of resources in the PCN. 2.4. Delegate, organise and prioritise autonomously to ensure the safe delivery of the service and making judgements in complex situations e.g. unexpected service demands 2.5. Ensures that all team and individual objectives are clearly defined within the PCN. 2.6. Takes responsibility for their own and others health and safety in the working environment. 2.7. Ability to make highly complex judgements, including in new or unfamiliar situations by analysing and interpreting a wide variety of complex information and data sources to compare options and to take action as appropriate (this may be clinical or non-clinical judgements). 2.8. Provides a leadership style which is underpinned by strongly held values around equality, diversity and openness; effectively builds and maintains relationships with direct reportee(s) and other key individuals across the PCN. 3.3. Demonstrates clinical leadership, initiative and creativity in developing projects inspiring others to be positive in their contribution to continuous improvement 3.4. Demonstrates a highly specialist knowledge of professional roles and responsibilities in order to explore and develop new and innovative methods of service delivery in order to meet current and new demand 3.5. By demonstrating highly developed change management behaviours, the post holder will lead complex projects, which may impact on own or other services, in order to implement key strategies and quality standards 3.6. Demonstrates resilience when responding to challenge, change and complex or difficult situations 3.7. Clearly articulates the expected clinical standards of the service, monitors and identifies where standards are not met and takes action to address holding people to account where necessary 3.8. Ability to represent the PCN at relevant local or regional clinical forums. 4. Learning and Development 4.1. Undertakes any training required to develop and maintain their proficiency in new and emerging techniques within the clinical and operational area and demonstrate competence within professional body requirements 4.2. Reflects on and evaluates own practice and identifies areas of development by setting appropriate objectives via appraisal and clinical supervision 4.3. Devises, delivers and leads specialist training and development programmes within team and to other services, teams or agencies 4.4. Provides expert advice related to clinical specialism to other practitioners, agencies and individuals 4.5. Ensures appraisals for direct reportees are undertaken in accordance with policy; manages performance which falls below standard in accordance with the appropriate HR policies and processes seeking support from appropriate others where required. 4.6.
