Work with nursing and scheduling staff to ensure all patients have appropriate appointments booked, and that pre-treatment investigations and procedures have been undertaken with results available before treatment. Ensure prescriptions have been completed and results of appropriate investigations have been recorded prior to commencing SACT Triage incoming calls and initiate appropriate response in accordance to assessment tools, protocols and individual pathways, liaising with the SACT nurses and CNS as appropriate. Make appropriate telephone calls to patients when required, Informing patients of relevant appointments, providing basic telephone advice and refer on or signpost to other sources of support. Ensure any investigations and diagnostics are booked promptly eg, blood tests, ECHOs and PICC line insertions, and district nurse referrals completed where necessary Co-ordinate patient documentation, including investigation results, prior to and during their attendance at the SACT treatment unit Proactively identify emerging issues that may cause delays at any point in patient pathways and take action to improve administration or any other aspect of the SACT pathway. Escalate to the nurse-in-charge where necessary To have knowledge and understanding of all of the cancer pathways and have an active role in redesigning pathways and service improvement, particularly in the analysis of existing pathways To proactively monitor patients progress along their pathway, highlighting problems to the SACT unit managers / Lead Chemotherapy Nurse and escalating where necessary according to local protocols. Negotiate with and instruct relevant personnel, departments of clinic bookings/diagnostic tests/admissions that need to be arranged or bought forward to ensure prompt treatment. To understand SACT service provision for cancer patients within the ICS, the steps along the patient pathway and the associated flows of information. This will include referral processes, diagnostic and treatment options and involves using the Trusts computer systems (CLIP, ICE, WINPATH, CRIS, PACs, OASIS, Bluespier, MOSAIQ, System One, LCMS).
