Key Responsibilities Main Duties Work with the Clinical Pharmacists and Clinicians in accordance with agreed practice prescribing standard operating procedures. Process repeat prescription medication requests, including conversion of acute requests repeat medicines, changing quantities in line with agreed protocols & standard operating procedures. Conduct medication reconciliations post discharge & hospital attendances, including synchronizing medications. Liaise with secondary & primary care colleagues to ensure correct medicines are continued following transfer of care. Undertake patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients. Undertake medication reviews for a defined cohort of patients in line with the practice recall system & medication review processes and ensuring appropriate monitoring is in place. Support the clinical pharmacists and clinicians in structured medication reviews (SMRs) and medicines reconciliation. Collaborate with the Clinical Pharmaciststo support the delivery of ongoing medicines optimisation objectives, including (but not limited to) structured medication reviews (SMRs), reducing polypharmacy, improving antimicrobial stewardship, and safe opioid prescribing. Support patients to take their medications to get the best from them, reduce waste and promoting self-care. To develop and maintain medicines related communications to all practice staff. Undertake audits around patient safety and cost effective prescribing and participate in quality improvement initiatives. Provide a contact for patients and colleagues with medication queries. To reduce the medicines related workload on clinicians. Increase safety and quality of prescribed medicines. To ensure efficient medicines optimisation processes are being followed. Oversee efficient ordering and return processes and reducing medication wastage To assist in reducing medication errors such as implement changes to medications that result from MHRA alerts, product withdrawal and local & national guidance. Promotion of Electronic Repeat Dispensing (eRD) and online ordering. Develop relationships with other pharmacy professionals and members of the multidisciplinary team to support integration across health and social care including primary care, community pharmacy, secondary care and mental health. Support practice teams in streamlining general prescription requests. Triage clinically complex queries from pharmacies and patients. Assist in the delivery of the practice covid & influenza vaccination programmes. Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. To contribute to the delivery of the highest possible quality standards, the NHS Long Term Plan, Quality and Outcomes Framework, Investment and Impact Fund and locally commissioned enhanced services. To support the practice in reviewing and developing practice policies for CQC requirements. Contribute to clinical governance, safety, and risk management by monitoring prescribing trends, reporting safety incidents, and supporting the implementation of learning from significant events and patient safety incidents, in line with practice processes. Maintain up-to-date CPDrelevant to the role and in accordance with GPhC requirements. To develop a culture that promotes equality and values diversity.
