Effectively manage the community dialysis cohort in conjunction with the named consultant incorporating holistic assessment, appropriate and timely referrals, educational and professional issues Ensure that standards of home peritoneal dialysis care are developed and maintained in line with national standards. Perform comprehensive assessments of patients nursing needs, providing and delivering a high standard of nursing care by initiating and co-ordinating the assessment, delivering medications along with planning and evaluating care. Timely, problem solving and corrective action for peritoneal dialysis patients including fluid prescription changes, liaising with GP surgeries for medication requests. The post holder will be required to ensure that the service is covered 7 days a week, and to work flexibly to meet the demands of the service (some short weekend shift will be required). The post holder will be based with the Peritoneal Dialysis team at the dialysis unit, but the post holder will be required to work across all hospital sites and within the community in patients and nursing homes. Attend consultant clinics as per prior agreement, identifying and accepting appropriate referrals, monitoring patients progress and medical conditions where appropriate in conjunction with the consultant. Provide a holistic assessment of community dialysis patients and a comprehensive evaluation of the factors that impact on treatment modality choice. This will include home visits, education sessions, assessing effectiveness of patients peritoneal dialysis therapy, ensuring fluid prescriptions are accurate and signed by prescribers in a timely manner, taking/monitoring/interpretating blood and test results, implementing changes if required and ensuring subsequent follow up clinical appointments are made in a timely manner. Working within the guidelines of the National Service Framework, Provide prompt communication to the relevant, consultant and the team, of the outcome assessments and the patients chosen treatment modality, ensuring that both medical and patient concerns are assessed and treated accordingly.
