An exciting opportunity has arisen for a Band 7 Senior Practitioner to join the Neighbourhood Mental Health Team in Salford, leading on work with patients presenting with mental health and difficulties in the Ordsall and Claremont neighbourhood. The successful candidate will work as part of the Neighbourhood Team leadership team and be an experienced, driven, and passionate individual, able to work collaboratively with both patients and wider agencies, to ensure that people with complex presentations are able to access the right care to meet their individual needs. The successful candidate will have a proven track record in the area supporting people with mental health conditions, demonstrating comprehensive assessment, risk formulation , multi-agency working and care planning skills.

1. Provide clinical leadership, supervision, support, and consultation to a multi-agency Team in Living Well. The post holder will be based in the Ordsall and Claremont borough of Salford but will also offer clinal support to the wider team when required. Use of a car is desirable to role.





2. Undertakeliaisonanddevelopeffectiveworkingrelationshipswith GMMH and statutory and non-statutory partners, supporting the Partnership huddle established within Salford.



3. To maintain own clinical and professional competence.



4. To actively participate in Group Supervision and other supervisory/supportive forums.



5. To participate intheservice bronze on callrota.



6. Deputize for the Team Managerinhis/her absence and take on any delegated duties asrequired.



7. To apply trauma informed approaches and interventions where necessary.



8. Participate in the Hub Worker role.



9. Manage and evaluate a complex caseload of service users with co-occurring conditions in line with relevant clinical pathway as outlined within the operational procedure and will include: -

a) Assessment

b) Formulation

c) Therapeutic intervention as required.

d) Completion of ReQol and Dialogue +

e) Community Connections and onward journey

f) Close working with all colleagues across the system when applicable

g) To be responsible for a complex caseload
