Qualifications and Experience Registered (first level) Nurse Current Registration on NMC Register Desirable - post registration experience working in primary care Evidence of Ongoing Professional Development. Key Features of the Role Delivery of nursing care to patients in the practice population Direct Clinical Involvement in Long Term Condition and Chronic Disease Management Providing advice, support and liaison with other practice staff Clinical Nursing Practice Complex Wound Management and provide support to junior staff members to create wound care plans. General support to junior staff members Vaccinations and Immunisations Infection Prevention & Control and assist with audits Emergency Management i.e. anaphylaxis and resuscitation Assess, plan, provide and evaluation nursing care to meet the care needs of individuals and groups in the practice population. Undertake diagnostic health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy Contraception Women's Health including cervical screening, swabs and HRT Management of Long Term conditions Diabetes Respiratory Disease Management-COPD & Asthma Cardiovascular Disease Management-CHD, Stroke/TIA, PVD & Heart Failure Must have business insurance to attend home visits to conduct LTC management and immunisations Enable individuals with long term conditions and their carers to make informed choices concerning their health and well-being and to organise their own support, assistance and action. Must have business insurance to attend home visits to conduct LTC management and immunisations Travel Vaccinations Support individuals with long term conditions and their carers to change theirbehaviour and to reduce the risk of complications Manage programmes of care for patients with acute and chronic diseases by planning and evaluating care Work across boundaries and in partnership with primary / secondary care clinicians and social services, coordinating care and promoting a multi-disciplinary approach Review prescribed medication and provide advice on all aspects of medicines management. Refer to relevant clinicians where appropriate Contribute to the development of practice policies and system of practices
