Up to date understanding of National Hospital Discharge policies and guidance Ability to manage, and adapt to change within multiple teams Ability to contribute to, monitor and implement changes and improvements to services Knowledge of safeguarding policies and procedures Ability to interpret clinical information to support risk assessments when planning discharges Excellent communication skills, both verbal and written, and the ability to influence others in clinical decision making towards discharge Able to change priority, often at short notice Creative approach to system pressures Sound conversational skills allowing for difficult conversations with both patients, families, and staff Ability to remain calm and consider wider picture in particularly stressful situations. IT proficiency Ability to prioritise workload, often in response to a changing picture Ability to work autonomously as well as part of multiple teams This post will require onsite presence at an acute hospital Willing to work in other areas of HCRG as and when required to do so Attendance at and participation in wider system meetings Desirable Teaching and assessing in practice qualification Experience of quality improvement work, in particular with regard discharge planning Ability to influence and negotiate
