Crynodeb
We are excited to share an opportunity to join the West Ipswich Care Management initiative which supports high users and predicted high users of acute non-elective beds. Care Management is a way of intensively supporting this group of patients with proactive care co-ordination and additional GP & Community Matron capacity. We have started this service in the West Ipswich Integrated Neighbourhood Team area and our aims are: To improve the care of these complex patients Ease workload pressures across the system Reduce hospital admissions
