To deliver high-quality social work services within a hospital discharge team, ensuring safe and timely discharges under the Discharge to Assess (D2A) model. The role involves working collaboratively with NHS colleagues and community services to meet statutory duties under the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks.
Coordinate safe and timely discharges from acute and community hospitals.
Implement D2A pathways and liaise with health professionals.
Complete Care Act assessments and reviews.
Develop person-centred care plans promoting independence.
Undertake Mental Capacity Assessments and contribute to Best Interest decisions.
Respond to safeguarding concerns promptly.
Attend MDT meetings and liaise with NHS discharge coordinators.
Engage with families, carers, and advocates to ensure informed decisions.
Maintain accurate records on electronic case management systems.
Adhere to local authority and NHS policies.

2+ years of post-qualification experience.
