Crynodeb
Main responsibilities include: To be an integrated team member within the ward team, and Southwark OT team to address occupational and recovery goals for patients. To undertake occupational therapy assessments for a designated caseload, assessing health and well-being from an occupational perspective. In collaboration with the multi-disciplinary team and service users, to plan and implement individual or group therapeutic interventions using graded activity to achieve therapeutic goals which support recovery. To utilise outcome measures that give clear evidence that occupational therapy has supported individual recovery and social inclusion. To apply a high level of understanding of the effect of disability and provide training and advice on lifestyle changes and adaptations to the service users social and physical environment. Where applicable, to assess and prescribe small aids and equipment and recommend adaptations to maximise independence for service users in home and community environments. Being active in discharge planning, providing occupationally focussed recommendations and working closely with discharge coordinators, care coordinators, wider involved agencies and families/carers to ensure transition into community is done in the most effective and person centred way.
