Empower people to realise and maintain their potential in relation to health and well-being. Under supervision and guidance contribute to support plans for people with learning disabilities to enable the proactive management of identified health needs which may include challenging behaviour. Under supervision and guidance, we contribute to the delivery of a wide range of therapeutic interventions ensuring evidence-based practice according to the National Institute for Clinical Excellence (NICE) guidelines and National and local procedures. To participate and contribute to multidisciplinary case discussions. Support and contribute to the development of accessible person-centered support plans and information in partnership with service users, carers and the multi-disciplinary team. To be able to adjust communication approaches to meet the complex needs of clients with varying levels of learning disability. To demonstrate positive interpersonal and excellent communication skills, both written and verbal To be aware of the clients clinical condition and individual circumstances as necessary to perform duties.
