The Independence at Home Team are seeking to appoint an enthusiastic and motivated individual to the role of Lead Care and Support Worker.
The successful candidate will lead by example by supporting their colleagues in delivering person centred care and support as well as reviewing care and support plans and Risk Assessment and supporting with the setting of rota’s. Although there is no direct line management responsibility, this post provides an excellent opportunity for career progressions towards a supervisory/management position.
The post holder will be expected to divide their working week as follows: two days in South Denbighshire, one day at Gorwel Newydd - Rhyl, one day at Nant Y Môr - Prestatyn, and one day with the Reablement Team in North Denbighshire.
In addition to supporting with the management of the service, the post holder will be expected to deliver direct care and support on a regular basis as well as offering shift cover for sickness and other unplanned absences, this may be within a fixed site setting or within the community. The post holder must have a flexible attitude to their working week and will regularly work weekends and cover night shifts.
