HCSW to work with a wonderful hard working team with a great sense of humour on a very busy Frailty Assessment.
Our patient are assessed and after 12 to 24 ours hours are discharged or admitted to another ward. Many of patients are admitted with different medical conditions so every day is different.
· Supports patients and their families, through communicating information using a varietyof methods,toensureeffectiveunderstandingandreportingtheoutcometo the nurse in charge.
· Contributes to theeffective communicationbetween membersof the multi disciplinaryteam,takingpersonalresponsibilitytoshareupdateandshareinformation.
· Usesknowledgeandexperiencetodeliverroutinetasksasagreedwiththe registered nurse
· Usesknowledgeandexperiencetofollowagreedpolicyandprocedure
· Undertakesarangeofpredefinedprocedures,forwhichspecifictraininghasbeen given under the direction of the registered nurse
