Work in accordance with COT & HCPC rules of professional conduct and within local care pathways, policies and standards. Professionally and legally responsible and accountable for all aspects of his/her own professional activities and to manage clinical risk within own caseload at all times, whilst remaining responsible to Team Leader/Clinical Specialist. Promote awareness of the role of Occupational Therapy within the IIDT. Work as an autonomous member of the multidisciplinary team, contributing to team discussions and influencing decisions about patient care and discharge pathways To represent IDT at the board rounds, proactively driving discharge, questioning discharge plans, predicted discharge dates and escalating delays to managers. Hold responsibility for own caseload assessing their health and social care needs, to ensure the correct level of support is organised for discharge. To review patients on active caseload and amend discharge plans as required seeking senior level of support as required on tasks outside of identified competency. Carry out assessments using a variety of modalities, this may include physical, cognitive, perceptual and psychological intervention, recording all outcomes and liaising with community services to support discharges. To identify and order the equipment required to support discharge for those patients case managed by IDT. To effectively communicate the discharge process and plans to patients / families and carers, leading on discharge planning meetings as required.
