About the Role

As a Specialist Navigator, you will offer early intervention and ongoing support to prevent individuals with multiple and complex needs from returning to rough sleeping following a discharge from hospital. • Knowledge of local social and healthcare services and an ability to navigate systems and pathways into services
  • Experience of supporting individuals with complex needs (e.g. rough sleeping, substance misuse, physical and mental health needs)

  • Evidence of working successfully in partnership across different sectors, building and maintaining good working relationships
  • Ability to advocate and negotiate on behalf of citizens and to give independent advice
  • Knowledge of housing legislation, rights and responsibilities in both the public and private sectors
