Summary
Chiswick PCN are looking for a PCN Project Manager on a permanent basis. The PCN
Project Manager will support the PCN with a wide range of business, data submission,
transformation and improvement initiatives across the PCN. This post will be based on
site across the six PCN member practices.
The postholder will be instrumental in progressing the PCNs development plane. This
role will work with member practices to break down challenges to implementation
across the patient population.
