To provide expertise within the Nurse-led Cardiac Rehabilitation assessment clinics and on the Cardiac Rehabilitation programme. Reviewing referrals to the Cardiac Rehabilitation Service and signposting them to whichever programme best meets their needs but maintains their safety. Ward visits Ensure an approach to care which recognises the unique needs and views of the individual patients and their carers which places the patient at the centre of all activities.
