Are you interested in influencing clinically and cost-effective care across South East London (SEL)? Do you have a keen eye for detail, the ability to think critically and the motivation to develop your knowledge of medicines optimisation policies?

If so, you should apply for the role of Joint Formulary and Medicines Pathways Pharmacist. This role will require you to combine your well-developed clinical pharmacy knowledge with advanced critical appraisal skills. You will be responsible for assisting in delivery of a formulary service across SEL ICS, including GSTT, Kings College Hospital and Lewisham and Greenwich Trust. You will gain significant experience in preparing evaluations, assisting in the development of local medicines treatment pathways and presenting to senior decision makers including consultants and Chief Pharmacists. You will be a key member of both the SEL Joint Formulary Committee and the SEL Integrated Medicines Optimisation Committee. The role will also include a level of clinical commitment, and support for the Medicines Information Department for work relevant to formulary and interface matters.

• To support implementation and development of formulary services within the Trust, at KCH and LGT and within the wider health
economy of the South East London Integrated Care System (ICS).
• Prepare and distribute the agenda for Joint Formulary Committee (JFC) meetings, including coordinating agenda items with attendees
from the 3 acute Trusts as appropriate.
• To perform new medicine evaluations, covering efficacy, safety and financial implications.
• Assist with the maintenance and development of the South East London Joint Medicines Formulary website.
• To answer formulary related enquiries, originating from GSTT and those referred to the formulary team from KCH and LGT.
• To assist with the review and clinical evaluation of unlicensed medicines requests and individual funding requests.
• To assist with the implementation of new medicines within the Trust and SE London ICS, e.g. contributing to updating medicines
treatment pathways.

Department Information

Guys' and St Thomas’ Pharmacy Department is one of the largest in the UK and has a strong focus on individual patient care. Our pharmacists work across multi-disciplinary teams both in the hospital and community setting, to deliver advice, guidance, and support within a culture of medicine safety. The focus is on patient-centred care and this is achieved through providing optimised pharmaceutical care for patients as part of a ward-based and clinical pharmacy service and extending pharmaceutical services beyond traditional boundaries. This includes attendance on consultant-led ward rounds and contributing to multi-disciplinary meetings.

The pharmacy Medicines Information, Governance and Formulary teams are based at Guy's hospital but deliver numerous programs of work and services across all the hospital sites.

Leadership
• To develop and maintain effective working relations with colleagues in primary and secondary care within the locality of GSTT, KCH, LGT and the local ICB
• To encourage and support initiatives by others in improving medicines optimisation at GSTT

The South East London Joint Formulary Committee (JFC) and Integrated Medicines Optimisation Committee (IMOC)
• Prepare and distribute the agenda for JFC meetings and to arrange the venue for JFC meetings
• Assist with the coordination of JFC agenda items with attendees from 3 acute Trusts as appropriate
• Coordinate and prepare JFC papers, minutes, and the annual report
• Preparation of JFC and IMOC drug evaluations and JFC outcome letters before circulation
• Assist with the consultation process for formulary submissions and other JFC and IMOC agenda items across the acute Trusts
• Update and publish a record of ratification of NICE and other nationally approved medicines for GSTT, KCH and LGT
• Prepare a monthly JFC report to the GSTT DTC and the equivalents at KCH and LGT including information as to where local internal action or review may be required
• Manage the JFC circulation list, and declaration of interests forms
• Work with the Paediatric Formulary Committee, under the direction of the Principal Pharmacist: Joint Formulary, Regional Medicines Pathways and Interface
• To provide assistance and advice relating to new submissions and queries for the Paediatric Formulary

Formulary and regional medicines pathway maintenance and support
• To support implementation and development of formulary services within the pharmacy department, the Trust (including community services and outpatient pharmacy), at KCH and LGT and within the wider health economy in SE London Integrated Care System (ICS)
• To assist with the production of a monthly email newsletter relating to formulary and regional medicines pathway updates, disseminated to acute Trusts and SE London ICS
• To answer formulary related enquiries (from clinicians, pharmacists, technicians, nurses, primary care practitioners and pharmaceutical representatives), originating from GSTT and those referred to the formulary team from KCH and LGT.
• To assist with the review and clinical evaluation of non-formulary requests, unlicensed medicines requests and individual funding requests originating from GSTT, as well as those referred for review from KCH and LGT
• Assist with supply issues and product discontinuations affecting GSTT, KCH and LGT
• Assist with the maintenance and development of the South East London Joint Medicines Formulary website
• To assist with the implementation of new medicines within the Trust and SE London ICS, e.g. contributing to updating medicines treatment pathways.
• Manage formulary related decision making for the Principal Pharmacist: Joint Formulary, Medicines Pathways and Interface Management in their absence

Education and Training
• To contribute specialist knowledge to the wider healthcare environment.
• To identify and meet training needs of other staff and students of all disciplines in order to undertake the roles required of them with respect to use of information about medicines and formulary services
• To advise others about the legal and ethical issues affecting the formulary services
• To identify own training needs
• To actively seek out opportunities to meet own training needs
• To evaluate the results of training and re-define own needs as necessary

Clinical Practice
• To act as a clinical role model for junior pharmacists and demonstrate the ability to provide safe, clinically effective and cost efficient use of drugs in selected groups of patients.
• To participate in the pharmacy service at GSTT at practitioner level as appropriate
• To demonstrate specialist knowledge in the evidence behind and considerations required for the use of unlicensed medicines within the Trust
• To ensure that appropriate Pharmacy and other clinical staff are kept aware of the relevant legal, ethical and professional aspects of unlicensed medicines use
• To maintain a broad level of clinical practice
• To train other pharmacists, doctors and nurses to practise in accordance with Joint Formulary recommendations
• To provide other duties as specified by the Lead Pharmacist for Medicines Effectiveness and Safety
