Community Lead Nurse (RMN/RNLD)
Radis Community Care is looking to recruit a Registered Nurse (RGN/RMN/RNLD) to join our team of community nurses specializing in complex needs in the community. This role may require extensive travel. Primarily looking for a RMN with a speciality in LD. Our clients have clinical as well as mental health nursing needs and previous experience in this field or an RGN qualification would be advantageous.
What you'll be doing
You will be part of a compassionate team who are committed to our clients wellbeing. We take a personalised approach, using proven therapies to help people move forward with confidence and build a healthier, more fulfilling future. As a community nurse, you will play a key part in their recovery, assisting support staff with training and competencies. Writing support plans, risk assessments a knowledge of ACP would be advantageous, undertaking routine tasks and activities as directed or delegated to facilitate wellbeing, dignity and treatment of patients.
• Deliver prescribed care to own allocated clients
• Assess, plan and deliver with the client and their carer/family, nursing care which takes account of clients choice and wishes and acting as the clients named nurse.
• Accurately observe and engage with clients in order to assess their need and evaluate their progress.
• Monitor and review Nursing support plans.
• Evaluate and respond to changes in patient conditions and needs.
• Attend MDT meetings and be able to confidently discuss own clients.
• Be confident to liaise with health professions with regards to client’s needs.
• To have strong and proficient computer skills.

Skills & Qualifications required
The role requires a qualified Registered Mental Health Nurse (RMN/RNLD) with 6-12 months of post-registration, RGN Qualification highly desirable, active NMC registration with no restrictions. Candidates should have a solid knowledge of NMC guidelines, requests, and professional practices, along with evidence of continuing professional development. A positive attitude and a strong commitment to change, improvement, and quality are essential.
