Crynodeb
Our community hospitals, called Intermediate Care Units, support inpatients with short-term recovery and rehabilitation. We aim to provide a service that supports patients to live independently and return home when it is safe for them to do so.
Kleinwort Intermediate Care Unit provides short term rehabilitation for individuals over 18 years of age. Individuals are referred for rehabilitation following an acute worsening in their health such as infection, an acute hospital admission following illness or surgery, a fall or an adjustment of complex medication.
The candidate should be friendly and be able to communicate well with both patients and colleagues in a professional manner to support and maintain a positive attitude and outlook within the multi disciplinary team.

a) To report variances in a patient’s condition immediately to a relevant professional.
b) To maintain standards of cleanliness of ward environment and patient bed area.
c) To undertake nursing tasks under the direction of a qualified practitioner i.e, catheter care.
d) To work with other team members to maintain adequate nutrition and hydration of patients.
e) To work within a team supporting and assisting qualified staff in the implementation of patient care (to include personal hygiene, toileting, dressing, moving and handling, mobilising, making beds, serving meals and feeding patients.
a) To adhere to patients individualised care plans as prescribed by the qualified staff and other multi professional team members.
b) Perform simple wound care, ECG recording, continence re-assessment, stoma care, catheter care, venepuncture and other clinical responsibilities as delegated by qualified nursing staff.
c) To ensure correct use of equipment
d) To maintain accurate records
e) Safely handle patients’ property and valuables
f) Report complaints or concerns to nurse in charge/shift leader.
g) Act as ambassador/role model for the organisation
h) Contribute to policy development as required.
