We have a great opportunity for two Support Coordinators on a full-time, permanent basis in our Early Intervention and Prevention Team and our Hospital Discharge Team.

About the team...
As a Support Coordinator, you will be part of a large, supportive and truly integrated multi-disciplinary team, including Occupational Therapists, Physiotherapists, Nurses, Social Workers and Support Coordinators. Our team works closely across services, with some members employed directly by the NHS, reflecting our strong partnership approach.

We work closely with the people we support to help them maximise their independence and remain in their own homes for as long as they choose, with reduced reliance on external services. Our team plays a key role in facilitating timely hospital discharges and preventing unnecessary admissions.

We achieve this through a range of support including Intermediate Care, Goal-Led Therapy, nursing input, innovative assistive technology solutions and effective signposting to our local voluntary sector. We also provide dedicated support for individuals with sensory loss, including access to our assessment suite where they can trial specialist equipment.

Key responsibilities...

Apply a strength-based approach to connect people to their community/social network and problem solve together.

To visit people in their home environment or in hospital, carry out assessments or signpost to where support can be provided.

Work with people, including advocates and carers, to develop person-centred plans, designed to meet needs in ways that maximises independence, choice and control and are legal and affordable.

To make phone calls to the public and professionals – recording information, referring to the correct person or team. Signposting, offering advice and information.

Complete Trusted Assessor Training, so you can assess for and prescribe basic equipment to promote independence.

Interview date
Interviews will be held on Wednesday 3rd December 2025.

Interviews will be conducted in person, providing candidates with the chance to personally meet the team and experience our work environment.
