Provide expert, autonomous clinical assessment, diagnosis, and treatment for patients presenting with a wide range of conditions. Initiate and interpret diagnostic tests and investigations. Prescribe medicines safely and appropriately under the NMC prescribing standards. Provide long-term condition management e.g., diabetes, COPD, asthma, hypertension. Deliver urgent care and triage same-day appointments. Undertake home visits or care home reviews as required. Promote self-care, prevention, and health education. Identify, manage, and refer safeguarding concerns appropriately. 2. Clinical Leadership and Teamwork Support the development of clinical protocols, pathways, and best practice standards. Provide mentorship, clinical supervision, and support to junior nurses, nursing associates, and HCAs. Contribute to multi-disciplinary team meetings and case discussions. Promote a culture of teamwork, continuous learning, and clinical excellence. 3. Quality, Governance, and Safety Ensure clinical practice is aligned with NMC Code, NICE guidance, and local/national policies. Participate in audits, significant event reviews, and quality improvement initiatives. Maintain accurate, contemporaneous patient records in line with GDPR and data protection regulations. Contribute to CQC compliance and preparation for inspections. Report and manage clinical incidents and complaints in accordance with Practice policy. 4. Education and Professional Development Maintain clinical competence through continuous professional development CPD and revalidation. Participate in in-house and external training programmes, maintaining evidence of learning. Support the training and education of students and other members of the primary care workforce. Engage with appraisals and reflective practice to enhance personal and professional growth. 5. Service and Organisational Development Contribute to the development and implementation of new services and models of care within the Practice or PCN. Support QOF achievement and delivery of enhanced and locally commissioned services. Participate in clinical research, audits, and service evaluations where appropriate. Work collaboratively with the PCN and wider health and social care teams to improve patient pathways.
