We are delighted to offer an exciting opportunity as a Healthcare assistant for the Hospital at Home team.

If you a passionate and dynamic Healthcare professional looking for a new challenge, we are looking for a healthcare assistant to be part of the Hospital at Home service. This is a combinations of community and acute working.

The post holder will work within the Hospital at Home team to contribute to the ongoing assessments of patients. Providing and monitoring patient care in line with the service pathways as agreed with supervising clinician(s).

Monitor the condition and health needs of people within their care on a continual basis in partnership with the wider Hospital at Home team.
Establish and maintain effective communication with patient/carers to enable an environment to support therapeutic relationships.
Visit patients at home to review progress against established pathway.

Escalate concerns to the clinical team where required. To provide home visits to obtain blood samples, capillary blood gases, ECGS, delivery equipment and other clinical tasks required for the patient. When required to set up patients on remote monitoring.
• Clinical community visits
• Performing Venepuncture, Capillary blood gases and ECG's.
• Assisting in remote monitoring of patients.
• Working alongside the MDT.
• Provide compassionate, safe and effective care and support to patients in a range of care settings.
• Monitor the condition and health needs of patients within their care on a continual basis in partnership with patients, families and carers, ensuring appropriate escalation to a registered nurse when required.
• Support patients to improve and maintain their mental, physical, behavioural health and well-being.
• Promote comfort and well-being by ensuring that patients’ personal and social needs are met and be able to care for a patient’s nursing needs.
• Provide and receive sensitive information concerning a patient’s medical condition.
• Provide effective evidence-based care for service users under the supervision of registered nurses in order to perform non-complex activities, in line with clinical governance and professional standards.

Changes to the UK immigration policy introduced on the 9th April 2025, increased the minimum salary threshold for certificates of sponsorship applications for Health and Care Worker Visa from £23,200 (£11.90 per hour) to £25,000 (£12.82 per hour). As per Agenda for Change, if you are new to the NHS your starting salary would be £24,937 (minimum Band 3 salary point from 1st April 2025 in line with National Living Wage requirements). This salary point is below the new minimum threshold, and therefore not eligible for sponsorship.
