Job Description

This post will formulate part of Aberdeenshire Health & Social Care Partnership’s Discharge to Assess model of care delivery. D2A targets patients with non-critical medical issues e.g. delirium, constipation, general deterioration. These patients often present at Assessment Units or Emergency Departments, and after necessary investigations can be discharged. The goal is to restore patients to their normal routines quickly, avoiding long-term admissions.

The Discharge to Assess (D2A) program is designed to facilitate a smoother transition for patients leaving the hospital. Instead of remaining in a clinical setting, patients are discharged to their own homes or other community environments, where they can be assessed for their ongoing care needs. This approach is part of the broader Home First ethos, which aims to provide care that is appropriate and tailored to individual needs, ensuring that patients receive the right level of support without unnecessary delays in their discharge.

The post holder will formulate part of the multi-disciplinary response to those patients who are eligible for a discharge to assess intervention

There is a responsibility for the post holder to demonstrate a commitment to quality service delivery through continuous improvement for the benefit of the Service and the organisation.

Requirements

This post is regulated work with Children and/or Protected Adults under the Protection of Vulnerable Groups (Scotland) Act 2007. The preferred candidate will be required to join the PVG Scheme or undergo a PVG Scheme update check.  Where an individual has spent a continuous period of 3 months or more out with the UK in the last 5 years, an Overseas Criminal Record Check will be required. You will be required to provide this check. A confirmed offer of employment and commencement in the post will be subject to the outcome of both these pre-employment checks being deemed satisfactory.

This post has a minimum requirement of two references, one of these must be your current or most recent employer.
