Surrey – Qualified Social Worker – Hospital Team - £35 - £38
Hybrid working – 3 days in the office
Essential – valid Driving Licence to drive in the UK and access to a car.
A typical caseload can can vary on any any given day. Typically, our Social Workers working in an Acute hospital setting may have a case-load of between 5-12. In our D2A function, the typical caseload is between 7-12. However, these are averages and not a hard and fast rule. What we can assure our workforce is supported accordingly management, ensuring our staff's caseloads are manageable, with practical assistance, direction and oversight provided by the relevant management.
- To assist our Surrey Heath D2A (Discharge to Assess function) transition residents off the D2A pathway in a timely way - To promote positive outcomes for our residents and their families - Undertake strengths-based work to promote personalised outcomes for our residents - Work collaboratively with Adult Social Care, Health, ICB and community partners - Working in a professional, co-productive and engaging manner with all stakeholders - Complete Care Act assessments as well as Mental Capacity assessments and undertake Best-Interest Decisions - Demonstrate excellent written, oral and recording skills - Have an ability to work independently and manage a case-load with oversight and support of management - Promoting safety and well-being of vulnerable residents through assessments and Safeguarding

Degree/Masters in Social Work and SWE registration.
2+ years of post-qualification experience.
Strong knowledge of UK social care legislation and practice.
