Are you looking for an excitingopportunityin a high performing and supportive team?Perhaps youare ready for your next challenge? We have a fantastic opportunity for an experienced NurseorParamedic tojoin theWokinghamUrgent Community Responseand Virtual WardTeamforan12monthsfixed term.
The rolewill provide clinical assessmentskills topatientsin their own home andin a carehome, who require an urgent response to prevent unnecessary admission to hospital.The post holderwillworkas part of a multi-disciplinary team to provideappropriate patientfocussed care.
The rolewill provideleadership and line management tojunior staffwithinthemultidisciplinary team, including nursesandsupport workers, and clinical supervisionandtraining/educationto nursesand support workers, asrequired.
You will be a team player and work collaboratively with staff from all disciplines whilstmaintaininghigh professional standards. You will ensuregood communication, and effective processes and procedures aremaintainedacross the service.
The post holderwill work37.5hrs per week,7days a week,between the hours of 8am – 8pm,which willincludeweekends.
• To practice asan advanced levelclinician, working as an autonomous practitioner within the multidisciplinary team including admitting to anddischarging patients from the Urgent Community Response Serviceand Virtual Wardin collaboration with theCommunityGeriatricianand GP.
• As anexperiencedpractitioner, you will take responsibility for the clinical assessment, diagnosis,treatmentand management of complex patients considering local and national guidelines.
• The postholder is expected to liaise effectively and professionally with all levels of staff, externalorganisationsand the public, dealing with confidential information inan appropriate manner
• To provide leadership and line managementto the Band6cliniciansandBand3 &4healthcare/multi-therapy assistants.
• Ensureaccurate, contemporaneous documentation and care plans aremaintainedaccordance with theNMC/HCPC, information governance and local / national guidance.
• Work collaboratively with colleagues and develop nursing and care delivery policies,identifyinghow policy willimpacton practice and service delivery.
• Work flexibly in response to service need across our multiple sites

• Registered General Nurse or Paramedic
• Advanced Historytaking andphysicalassessmentor working towards it
• Non-medicalprescriberor working towards it
• Able to work autonomously, with the abilityto work under pressure, with excellent organisational, communicationand time management skills to prioritise work
