✨ About the Role
Knowsley Borough Council is seeking two qualified Social Workers to join the Transition Team within Adult Social Care. This team supports young people aged 14–25, ensuring a smooth transition from children’s services into adult services.

You will play a vital role in preparing young people and their families for adulthood, undertaking Care Act eligibility assessments, developing and reviewing support packages, and helping young people settle into sustainable long-term accommodation or community placements.

Key Responsibilities
Conduct social care needs assessments for young people aged 14–25.

Develop, implement, and review tailored support packages.

Undertake Mental Capacity Act assessments and initiate COP DOL processes where required.

Collaborate with children’s social work teams, leaving care teams, adult services, and health colleagues (including learning disabilities, mental health, and ICB teams).

Ensure smooth transitions into adult services, with care plans in place before the 18th birthday.

Provide ongoing support to young people and families, promoting independence and stability.

Report to the Senior Practitioner and Service Manager within the Transition Team.

‍ Candidate Requirements
We are looking for experienced, qualified Social Workers with:

Proven experience in transition services and completion of SCNA to establish Care Act eligibility.

Strong knowledge of Mental Capacity Act and best interests processes.

Familiarity with Liquid Logic systems.

Excellent time management, engagement, and communication skills.

Confidence to challenge appropriately and ask the right questions.

Desirable experience includes:

COP DOL processes.

Adult safeguarding.

Trauma-informed approaches.

Transitional safeguarding developments.

Awareness of PBSS/active support models.

Working Arrangements
Office base days: Tuesday and Friday each week.

New starters will be expected to attend the office daily for the first two weeks to support induction.
