You are expected to have access to a car and willing to drive distances as we cover both Norfolk and Suffolk> Main duties of the job The ideal candidate will have some experience of working in the community supporting working age adults with mental health difficulties. You will need to have a sound understanding of the Care Programme Approach and be able to demonstrate your abilities in risk management.You will be part of the triage and assessment process managing referrals from our GP surgeries and other local services. We enjoy excellent close working relationships with all statutory and non-statutory partners in developing Recovery based care plans. You can expect to have plenty of opportunities for joint working, career development and training with a clear appraisal structure. Your role will be to: Undertake the assessment, planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment. Communicate decisions about mental health care services, referrals and future support arrangements to people with a mental health care need. Be responsible for accurate recording of all patients support, including assessments, reviews, day to day contact and discharges.
