Job Summary/Purpose


To support individualsreferred to secondary mental health services for assessment or ongoing care. Todemonstratethe Trust values ofinspiring hope, working together, to be caring and compassionate, to value and respect others/ourselvesand to be open and honest.

Your post will include working with individuals usingevidence-basedinterventions topromotetheir independence and recoverywhilstmaintaininghigh quality clinical documentation.


This includes respecting individuality, values, cultures and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.

• To assess, plan and review in line with requirements of theCare Act (2014) and s117 of the Mental Health Act (1983).

• To provide clinical supervisionto members of themulti disciplinaryteam based on your clinical and professional knowledge.

• To supervision junior members of staff in their clinical risk assessment and formulation of crisis and risk planning.

• To promote recovery-focused care and interventions for individuals.

• To ensure that all interactions and documentation is person-centredin line withNHS guidance ‘No decision about me, without me’.

• To co-produce individualised recovery plans with the individuals, carer, andothersidentifiedby the Service User.

• To promote andmaintaina learning culture within the team which supports the professional and effectiveness of clinical care.

• To contribute to the delivery of multidisciplinary training including the induction ofnew staff,studentsand trainees.

PracticeEffectively


• Todemonstratea high levelof active communication skills with individuals in avarietyof forums and situations.

• To utilise your highly developed interpersonal skills tofacilitateeffective communication. This includes identification and overcoming communication barriers such as sensory or cognitive impairment, hostile or highly emotive situations.

• To provide specialistmentalhealth interventions in the assessment, support,and recovery of individuals with severe mental health difficulties. These include but are not limited to Mental Stateassessments, Social Care Assessments,physical health care monitoring,side effect profile monitoring, etc.

• To ensure that all individuals have clear andaccurateinformationregardingtheir care or the care of their loved ones.

• Where required to act up in the absence of the senior practitioner.

• To seek out andparticipatein line management and clinical supervision.

• Ensure, as far aspracticable, the full involvement of individuals and theircarersin the assessment,formulationand review of their care plans.

• Tomaintainhigh standardsof clinical recording ensuring that information is alwaysaccurate,appropriateand up to date.

• Tocomply withGreater Manchester Mental Health NHS Foundation Trust’s policies and procedures with specific focus on confidentiality, Information Governance,and professional practice.

• Tomaintainyour personal well-beingusingall available resources available to you via the Trustorprofessionally, including PAM assist, Occupational Health, etc.

• Tocomply withall health and safety legislation and the welfare of yourself and the wider Trust.



Preserve Safety


• Toidentifyany unmet needs to support service improvement or planning to enhanceindividualsexperiences of health services within the Trust or in the wider health economy.

• To adhere to the principles of Safeguarding children and vulnerable adultsinlinewith Trust policy and local and national reporting procedures.

• With support from line managers, make safeguarding enquiriesin line with Section 42 of the Care Act (2014) ensuring that safeguarding is person-centred, proportionate,specific,and that outcome measures are achieved and risk is mitigated.

• To ensure that colleagues and junior staff are competent and compliant with Safeguarding legislation and guidance.

• To actinlinewith the Trust’s Infection PreventionandControl policies, procedures,and best practice guidance tomaintainhigh standardsof Infection Prevention and Control (IPCC)

• Tocomply withall service security requirementswith regarding to your own and others safety. These include lone working procedures,health and safetyat work,and risk management procedures.

• To ensure the management of riskin accordance withthe Trust Policies and procedures,including the investigation processin regard toRoot Cause Analysis, complaints,accidents,and near miss situations viatheInPhasesystem.
