Job Title: Advanced Clinical Practitioner Physiotherapist Ageing Well Team Role Summary: We are seeking an experienced and highly motivated Advanced Clinical Practitioner Physiotherapist to join our Ageing Well Team, based within Woodsley and Holt Park Primary Care Network. The role focuses on delivering physiotherapy interventions within a proactive care framework aimed at preventing deterioration, promoting independence, and enhancing quality of life for older adults living with frailty and/or dementia. The successful candidate will work collaboratively across multi-disciplinary teams to support complex patient needs through comprehensive assessment, treatment, and holistic care planning.
