Blackpool Community Mental Health Team is seeking to appoint a Band 6 Community Mental Health Nurse to act as key worker within a vibrant and diverse locality. Delivering care under the keyworker role, this post provides the opportunity for a forward thinking and enthusiastic individual join a multiagency team working across Blackpool catchment area. You will be passionate and committed to develop and implement safe, sound and supportive services that embrace the provision of holistic care.
You will be required to have a working knowledge of the assessment, formulation and management of risk alongside care with the ability to explore issues relating to the impact of health and social needs on mental health and well-being.
• To actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and code of conduct.
• To undertake mental health assessments including Risk assessments within a multi-disciplinary assessment team.
• To be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary.
• To provide a fast, responsive service to assess individual’s needs.
• Providing a communication structure between GP’s, IAPT and non IAPT services in LSCFT, non-statuary service, as a means of ensuring the highest possible standards of patient care
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
