Job Summary As a Prescribing Pharmacist, you will play a key role in medication management, ensuring safe, effective, and evidence-based prescribing for our patients. Your primary responsibilities will include prescription signing, medication reviews, reconciliation, and conducting safety searches to optimize prescribing practices and enhance patient care. You will work closely with GPs, nurses, and other healthcare professionals to support medication safety, chronic disease management, and prescribing initiatives, ensuring compliance with clinical guidelines and improving patient outcomes. Key Responsibilities 1. Prescription Management Sign and authorize repeat prescriptions in line with practice policies. Ensure prescriptions comply with best practice, local and national guidelines. Provide clinical oversight on high-risk medications and controlled drugs. 2. Medication Reviews & Reconciliation Conduct structured medication reviews (SMRs), ensuring medicines are optimized for patient safety and effectiveness.
