Key Duties: Prepare patients for on boarding to virtual platform- provision and explanation of equipment- BP machine, saturation probe, how to use DOCOBO application. To support with COPD discharge bundle completion and entering on NACAP COPD website. To be able to use the virtual platform and monitor alerts. To respond to enquiries, face to face and telephone, from GPs, patients and their relatives appropriately. To input accurate data into the computerised patient data information system and to provide accurate and up to date information relating to clinic lists using Oasis. To generate patient notes and letters, and other official correspondence, including self-generated non- routine correspondence if required. To deal with day to day incoming mail according to the office policy, assigning priority and/or taking any necessary action without reference to team staff if necessary. Preparation and maintenance of equipment as directed. Participate in the completion of clerical work as required by registered clinician To observe absolute confidentiality of all information obtained in the course of your work. To undertake any other duties within the grade in accordance with the needs of the service. General Duties: Patient / carer Support / Involvement: Provide education and advice for people with respiratory, helping them to understand the disease process, its management and potential coping strategies Having due regard for patient confidentiality, act as a resource for carers of respiratory patients, providing them with advice and support as required to improve their skills and confidence in the delivery of community respiratory care with support from registered nurses Personal / Professional Responsibilities: Be proactive in, and maintain their own professional development Maintain clear and concise records of the patient's management in accordance with Trust standards Promote and maintain good working relationships within and outside of the Trust, utilizing opportunities to promote the specialist service, and the work of the Trust Contribute to the creation of a care environment and culture that delivers a consistently high standard of patient-centred care Knowledge, Skills, Training and Experience Demonstrate the knowledge and skills applicable to post as detailed in the Person Specification Demonstrate planning and evaluation skills, and demonstrate ability to contribute to discussion to determine ongoing needs and treatment planning Prioritise workload effectively with support and supervision Demonstrate flexibility in the role. This may involve working across clinical pathways to support the needs of the service whilst ensuring the delivery of high quality care at all times
