Summary
Advanced Nurse Practitioner - Lewisham
A well-established, forward-thinking primary care team in South East London is seeking an experienced Advanced Nurse Practitioner (ANP) to join its friendly and supportive multidisciplinary team.
The practice serves a large, diverse urban population within a vibrant and culturally rich community. Based in a modern, purpose-built health centre, the team works collaboratively to deliver high-quality, patient-centred care while maintaining a positive and inclusive working environment.
The RoleThe Advanced Nurse Practitioner will work 6–8 sessions per week, providing expert clinical care and leadership within a skilled and supportive team that includes GPs, ANPs, a Clinical Pharmacist, Practice Nurses, and Healthcare Assistants.
The postholder’s responsibilities will include:

Participating in triage and duty sessions

Running face-to-face and telephone clinics for routine and complex health needs

Conducting home visits as required
-
Managing acute and chronic conditions across all age groups, including patients with mental health needs

Supporting service improvement and health promotion initiatives

Key Responsibilities-
Deliver advanced clinical assessments, diagnoses, and treatments for patients across all age groups

Undertake triage and on-call duties as part of the duty team
-
Prescribe and review medication in accordance with clinical and national guidelines
-
Lead care management for patients with long-term or complex conditions
-
Support disease prevention and health improvement activities
-
Participate in clinical governance, quality improvement, and audit programmes
-
Contribute to the education and mentoring of students and colleagues
-
Take part in leadership and service development within the nursing and wider clinical team
Person SpecificationEssential:
-
Registered Nurse with Advanced Practice qualification (MSc or equivalent experience)
-
Independent Non-Medical Prescriber
-
Experience working autonomously in an advanced clinical role
-
Competence in triage and managing patients across the lifespan, including those with mental health needs
-
Excellent clinical judgement, communication, and leadership skills
Desirable:

Experience working in primary care or general practice
-
Case management and nurse-led triage experience
-
Teaching or mentorship in a clinical environment
