Crynodeb
To provide dietetic support within an agreed caseload to individuals requiring treatment for HPB diagnoses. This will include those patients under the care of the Specialist Surgical HPB team. The delivery of this service can take place in outpatient clinics, at ward level, remotely via a benign MDT, and in other outpatient areas. To provide nutrition and dietetic support for adults receiving care under the specialty, and initiate, establish and monitor enteral feeding care for patients, and continued monitoring in the community, including interpretation of biochemical, anthropometric, and clinical condition. To provide a resource of nutritional knowledge for other health professionals (including dietitians), across the Trust and in other units that refer to the specialty MDT team. To work with patients managing their pancreatic enzyme supplements, advising on dose changes, once signed off as competent to do so by the HPB Consultants. Also, to liaise with medical staff to identify patients at risk of malabsorption. To support nursing staff and medical staff in identifying and managing patients requiring bile recycling To work with other professionals to ensure best practice. This will include reviewing, updating and implementing nutrition and dietetic-related policies and procedures. To work with Clinical Nurse Specialists within multi-professional follow up clinics To attend relevant multidisciplinary team meetings in order to take an active role in treatment planning for individuals and to facilitate good communication. This may occasionally include additional time required to attend a relevant meeting, but appropriate notice will be given, and time off in lieu will be granted. To liaise with surrounding Trusts, including CCGs, to provide a seamless and effective service. To assist in the remote management of patients with pancreatitis as part of the benign Pancreatitis MDT. To liaise with other healthcare professionals working in the specialty to assess teaching needs and to produce or purchase high quality literature for use by them. To play an active role in teaching other health care professionals including students. To support volunteer dietetic assistants To remain up-to-date within the specialty, and incorporate changes to assessment practices in response to internal and external initiatives. To audit working practices and service value. Methods that will be employed include continual review of present practice against service goals and documented standards e.g. NICE guidelines. To initiate and participate in regular survey / audit which will enhance the dietetic care of the acute medical and / or surgical patient To participate in data collection to measure activity and outcome To actively participate in nutrition-related clinical trials at national and local level, as required, within funded time. To provide input to departmental meetings and Journal Clubs and contribute to the strategic aims of the department.
