See attached JD & Person Spec for more detail To support the rehabilitation coordinator to deliver specialist assessments and treatment planning of critically ill patients requiring co-ordinated rehabilitation management. To support the rehabilitation coordinator to ensure patients receive an enhanced rehabilitation process for the entire patients journey through critical care to follow up in the acute hospital wards and community, ensuring continuity of care and seamlesstransitions between care providers beyond the acute hospital environment.3. Ensure patients have access to associated support services through appropriate and timely referral.4. Liaison with multi professional groups, patients, relatives and carers to ensure maximum effectiveness of assessment and treatment delivered.5. To be professional, legally responsible and accountable for all aspects of the work undertaken, including the management of the patients treated and to ensure a high standard of clinical expertise is delivered to those patients, maintaining up to date and accurate records as an autonomous practitioner.6. To support the rehabilitation coordinator in planning and development of rehabilitation services within critical care and be responsible for delegated projects.7. To act as deputy to the rehabilitation coordinator in his/her absence and be reportable to the clinical therapy manager and senior respiratory specialist.8. To participate in the on call rota.9. To contribute to the continuing professional development of other junior physiotherapists and students who work on critical care supporting their learning anddevelopment.10. To hold responsibility for own delegated complex physiotherapy caseload within critical care working without direct supervision11. To actively participate in supervision which will be achieved through regular informal and informal training and clinical reasoning sessions with the rehabilitation coordinator, peer review, case conferences.12. Supervise, educate and assess the performance of students; this would be to a graduate standard and involve working with universities to ensure the standard of practice and teaching meets the standards set by the degree level qualification.13. Undertake evidence-based audit as directed by the rehabilitation coordinator and if required research projects to further clinical practice and development of patient services. Make recommendations to the rehabilitation coordinator to suggestchanges to practice.14. Support the implementation of specific changes to practice or contribute to service protocols as directed by the rehabilitation coordinator.15. The post holder must comply with all relevant policies, procedures and training on infection prevention and control.
