Crynodeb
Lead on the assessment, planning, implementation, and evaluation of end-of-life care for patients within the PCN. Manage complex cases, providing specialist advice to primary care teams. Coordinate care with GPs, district nurses, hospices, and other community services. Support anticipatory care planning and facilitate discussions around advanced care planning.
