Crynodeb
The post holder will work as part of the multidisciplinary team. The post holder will contribute to assessment of both physical and mental health needs. Focusing on the wellbeing of those with serious mental illness, and demonstrating an understanding of the relationship between physical and mental wellbeing To work with other agencies to deliver a systems approach in ensuring patients mental and physical health wellbeing needs are met. The post holder will undertake annual physical health checks of patients who have been identified as having severe mental illness. They will take and record their blood pressure, pulse, weight, BMI, venepuncture for required blood tests. The post holder will arrange physical health assessments with the service user both at home, in clinics, and at GP surgeries; taking a proactive approach to engaging patients with SMI in the importance of annual physical health reviews. To provide information on health promotion including healthy eating, sport and exercise and smoking cessation To work with patients and relevant staff to develop care plans and complete the electronic physical health assessment form on electronic health record; ensuring the information is shared with patients and GPs.
