We are looking to recruit an experienced Advanced Clinical Practitioner or a Trainee Advanced Clinical Practitioner to the Mid Dorset Hub Team to deliver highly skilled care to the residents of Mid Dorset. If you are an Advanced Clinical Practitioner or a Trainee, we are keen to hear from you.

You will be based at Dorchester Local Office, Acland Rd, Dorset. You will join an excellent Multidisciplinary Team, who are supported by a Consultant Practitioner. As part of the Networks, you will join a multi-disciplinary Hub working in partnership with the local Primary Care Networks (PCN) delivering the Ageing Well Agenda

You will provide specialist clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and advanced decision-making skills. You will be working with the team in service delivery, triaging and prioritising visits to achieve our overall objectives in the delivery of Virtual Ward, UCR and anticipatory care. Your role will be community based, covering the Mid Dorset locality of Dorchester and surrounding areas.

The hours are 30 covering 8am-6pm with a weekend and bank holiday rota as the networks develop.



As an Advanced Clinical Practitioner within the Hub team you will conduct acute assessments to plan and manage acute episodes of care. You will undertake independent, comprehensive assessments which will involve complex clinical decision making to plan, implement and evaluate the care delivery according to changing healthcare needs and varying degrees of clinical risk.

You will be responsible for providing expert clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and advanced decision-making skills.

You will lead as a driver for change, to monitor and improve standards within the service through supervision, evidence-based practice, clinical audit, research, and education.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
