You will be able to find a full Job description and Person Specification attached within the supporting documents or please click "Apply now" to view in Trac The post holder will have a good standard of general education including numeracy and literacy skills, with a commitment to undertake training to QCF Level 2 in Health and Social Care/Clinical Healthcare Support. The post holder will be committed to complete Level 2 Vocational Training. They will maintain a personal portfolio and take part in reflection and appropriate learning from practice, in order to maintain and develop competence and performance. The post holder will be required to complete a competency framework workbook to demonstrate ability to undertake Level 2 vocational practices. The post holder will require standard keyboard skills to contribute to updating patients records electronically or paper copies. Communication: The post holder will : Develop communication skills in order to convey routine information effectively to patients, carers and other staff, overcoming any barriers to understanding and providing support during distressing or emotional events. Communicate clearly, effectively and appropriately with the multi-disciplinary team. Respond appropriately to routine queries, take phone messages and pass on written and verbal information to patients and staff in a timely manner. Provide appropriate information to patients, family/visitors/carers and colleagues. Accept instructions and receive constructive feedback from mentors/qualified staff. Give constructive feedback on patient care and activities to relevant healthcare professionals where required. Participate in discussions about patient care and development e.g. at team meetings, de-briefings, case conferences. Plan their own daily allocated workload within the area. Manoeuvre trolleys, wheelchairs and other equipment safely around the patients home. Identify potential risks for all staff, patients and visitors and adhere to relevant incident reporting procedures taking appropriate action/advice as needed. Following a period of training, will be required to undertake some routine observations for patients/clients under their care. Will require basic numeracy skills to utilise early warning indicators to recognise any patient deterioration/sudden change in condition, reporting immediately to Registered Nurse. Identify monitor and report other patient equipment alarms to the Registered Nurse. Responsibilities: The post holder will be required to provide personal care to patients/clients including assisting with feeding, bathing and dressing, promoting independence where possible. The post holder will assist in the care of patients with reduced mobility. The post holder will assist patients with personal care such as oral hygiene and foot care. The post holder will assist Registered Staff in undertaking last offices procedure. The post holder will attend organisational induction programmes, mandatory training sessions and annual updates. The post holder will collect/record data accurately in accordance with organisational guidelines. The post holder may be required to maintain adequate levels of stock, equipment and materials and facilitate the efficient, effective use of resources; reporting when availability falls below an acceptable standard or level to a Registered Nurse. The post holder will ensure safe custody, return or disposal of patients property, in accordance with organisational policies. The post holder will be responsible for the safe use of equipment used by self and others. The post holder will contribute to research, audits and clinical trials programmes carried out within the Department under direct supervision. They will be required to follow all legislation and organisational policies, and will be asked to comment on procedures for own area of activity. The post holder will work within established procedures when delivering care; and under the direct supervision of a Registered Nurse or Senior member of staff.
