To provide high quality nursing care to patients on the Sutton Health & Care (SHC) Reablement Unit as part of a cohesive Multi-DisciplinaryTeam. The post holder is responsible for holistic assessment and care needs of patients, including planning, implementation and evaluation. Inaddition, the post holder will be involved in health promotion and innovative activities thatenhancepatient servicedelivery andthe patient experience. The post holder will at times have management responsibility such as Nurse In Charge and supervising junior members of staff.
This post is not suitable for a newly qualified nurse.
Work within the Multi-disciplinary Team to provide direct patient care to a patients on Sutton Health and Care Reablement Unit.Takeresponsibilityforagroupofpatientsanddeliverhandsoncarewithinownlevelof competence.
Establishandmaintaineffectiverelationshipswithpatients,carersandthemulti-disciplinaryteamin ordertofacilitatepatientchoiceandindependence.

Clinical
· The post-holder will ensure that in all areas of practice they adhere to all current NMCguidance,andmustmaintainaccurate,comprehensiveandcontemporaneousrecordsinaccordancewithTrustandNMCguidelines,ensuringthatconfidentialityismaintained at all times
· Takeresponsibilityforagroupofpatientsanddeliverhandsoncarewithinownlevelof competence.
· To give skilled nursing care, based on principles of best practice and clinical evidence.and ensure that nursing care is patient focused and patient dignity and privacy aremaintainedat alltimes.
· Undertake comprehensive assessment of the physical, psychological and social careneedsofpatientsandprovidepersoncentredcareplansthatfacilitatethepatient’sownchoices,promotingindependenceand selfcare,as appropriate.
· Toensurethesafehandlingofbodyfluidsandcontaminatedsharps.
· Be alert to the needs of vulnerable adults including risk assessment and taking appropriateactionwhen required.
· Safelyadministerprescribedmedicationsandtomonitortheireffects.
· Usereflectivepracticeasamethodofensuringthatappropriateeffectivenursingcareisdeliveredto eachpatient.
· Ensurecompetencyandsafemaintenanceofallmedicaldeviceswithinthesphereofresponsibility.
· Workwithnursingandother healthcareprofessions,patientsandtheirrelatives/carerstoassist in the timely discharge from the clinical area through appropriate dischargeplanningin conjunction with theDischarge Co-ordinators.
Communication
· Establishandmaintaineffectiverelationshipswithpatients,carersandthemulti-disciplinaryteamin ordertofacilitatepatientchoiceandindependence.
· Toeffectivelycommunicatecomplexandsensitivepatientinformationtocolleagueswithinthenursingserviceandtomulti-disciplinaryteammembersinvolvedwithpatientcare.
· To maintain positive and effective communication and working relationships with patients,their families, and all other professionals to enhance a dignified and high standard of careprovision.
· At all times have an awareness of potential barriers to communication and anunderstanding of how to overcome these barriers to ensure effective communicationoccurs.Ensuredueregardisgiventothecustoms,valuesandspiritualbeliefsofpatientsandcolleaguesandthat equalopportunitiesare practisedatalltimes.
· CommunicateeffectivelythroughITsystemssuchasdatainputandwebbasedinformation.
ProfessionalLeadershipandManagement
· Superviseandsupportunregisteredmembersofstaffandall newmembersoftheteam.
· To organise and prioritise own work without direct supervision, reporting any issues,untowardclinicalincidentsorpersonallimitations,intheappropriatemannertotheseniornurseonduty completingnecessary documentationtolevel ofresponsibility.
· Attendandparticipateinall typesofward/departmentalmeetingsasrequiredincludingbeingthepatient’srepresentative atmulti-disciplinary meetings.
· Supporttheward manager and junior sister/charge nurseinmanagingtheresourcesrequiredintheprovisionofcare,withanawareness ofthe financial implicationsof allactions.
· Assistuntrainedstaffwithall clericalandhousekeepingdutiesincluding useofInpatientandclinical managementsystems
· Checkresultsandfileappropriatelyafterreviewbydoctorwhereappropriate
· Patientnotesmanagement.
· Take appropriate action in regard to any accident or incident to patient, staff or visitorwithinthescopeofthepostholder’sresponsibilityandinlinewithTrustguidelines.
· Toberesponsibleforthecareandsafekeepingofequipmentissuedforpersonalandteamuse and to report any defectorloss.
· Toparticipateinservicedevelopmentactivities,identifyingareasforimprovementasappropriate.
