- Assess patients physical, cognitive, emotional and social needs using appropriate assessment tools. Implement standardised assessments such as in MOHOST.

- Plan and implement individualised treatment programmes aimed at improving independence in activities of daily living.

- Monitor and evaluate the effectiveness of interventions and make gradual adjustments to care plans accordingly.

- Management a caseload and be involved in discharge planning for patients. Further to this, ensuring safe discharge by providing equipment provision, referrals to community services for support and completing home visits where necessary.

- Promoting independence, wellbeing and participation in meaningful activities in the ward environment and in the community.

- Working actively as part of a MDT (Multidisciplinary team) including physiotherapists, nurses, doctors, social workers and other allied health professionals.

- Attending MDT meetings, service development meetings, discussions around discharge planning and trainings that support your learning and development.

- Refer patients to appropriate services and liaise with external agencies e.g. social care, community OT’s and voluntary sector.

- Maintain accurate and up to date clinical records in line with legal and organisational standards.

· All interactions should be sensitive and responsive to the service users’ ethnic, cultural and social background, using a range of communication and engagement skills as appropriate to the complexity of the context.



· To evaluate referrals to the occupational therapy service for occupational therapy intervention, particularly identifying priorities for treatment intervention.



· To provide a comprehensive assessment, treatmentplanning, intervention and evaluation of the occupational therapy programme for the identified service user group that includes risk assessment with particular reference to service user need within the speciality.



· To analyse, grade and adapt individual and group activities to meet the occupational performance, environmental and vocational needs of service users.



· To monitor, evaluate and modify treatment for service users with multiple needs in order to measure progress and ensure effective intervention.



· To plan and implement appropriate discharge, transfer and case closure.



· To work with service users and carers as part of the multi-disciplinary team (MDT) to implement and evaluate the individual treatment plans in keeping with the relevant policy (e.g. CPA).
