Crynodeb
The Care Coordinator will play an important role within the Primary Care Network (PCN), working as part of the multidisciplinary team to deliver high-quality, patient-centred care. The primary focus of this role is to provide clinical support as a Health Care Assistant (HCA) undertaking tasks such as blood pressure monitoring, blood tests, ECGs, and other routine procedures. In addition, as part of their role, the post holder will provide secondary support to the Care Coordination team, assisting with administrative tasks, reception cover, and rota coordination to ensure smooth day-to-day operations across the PCN.
