Key Responsibilities:

Undertake assessments of risk and care needs for service users and carers, ensuring that appropriate action is taken, monitored and reviewed in line with legislation.

Manage a caseload of varying complexity, receiving and responding to referrals from multiple sources.

Conduct Community Care and Integrated Care Programme Approach assessments and risk assessments to protect the safety and welfare of individuals.

Complete statutory reviews, ensuring compliance with relevant legislation, policies and procedures.

Work collaboratively with service users, carers and other professionals to assess needs and agree support plans.

Engage with individuals who may refuse services but are at risk to themselves or others, using sensitive negotiation skills.

Manage crisis situations effectively, applying appropriate legislation and available resources.

Fulfil statutory responsibilities for the assessment of carers, including young carers, and develop carers’ care plans.

Prepare and present reports, assessments and statements to support decision-making across teams and agencies.

Maintain accurate, up-to-date client records in accordance with legislation, policy and procedures.

Facilitate needs-led service provision, working with providers in the statutory, voluntary and independent sectors.

Plan and agree service responses and resources, developing realistic care plans to meet individual needs.

Negotiate the purchase of care provision where appropriate.

Develop and maintain effective joint working relationships with primary care, housing, police and other agencies.

Establish and maintain strong professional relationships with service users, their families and other professionals to enable effective partnership working.

Essential Requirements:

Degree or Master’s in Social Work and SWE registration.

2+ years of post-qualification experience in Adult Services.

Strong knowledge of UK social care legislation, statutory responsibilities and safeguarding procedures.

Ability to manage complex caseloads and work in high-pressure environments.

Right to work in the UK.
