Crynodeb
You will work within an MDT. You will be line managed by the Health Team Manager, and clinically supervised by the B7 Clinical Lead Nurse. You will support and possibly clinically supervise a band 4 Nursing Assistant in the West LD team, based at Park View Resource Centre, Kings Lynn. You will develop working relationships with other key stakeholders from outside the organisation, including care providers and acute hospitals. You will communicate with all members of the Community Learning Disability Team and the wider Integrated Care System including acute and Primary Care Networks, to ensure a cohesive approach to care delivery. You will work in partnership with people with lived experience, including carers and family. You will work jointly with other professionals within a range of disciplines, including social care.
